1. POLICY

 

Zane State College provides benefits-eligible employees with life insurance and accidental death and dismemberment benefits and the College pays the premium cost. Please see plan document for details, guidelines, and restrictions.

1. POLICY GUIDELINES

This benefit ceases when an employee terminates employment with the College. The benefit can be converted and the employee may continue to self-pay for the benefit.
