3357:12-14-11-1-Replacement Diploma Procedure

A.PROCEDURE
This policy establishes the procedures for requesting a replacement diploma from Zane State College. It outlines the process, payment requirements, and conditions for issuing replacement diplomas.
B.PROCEDURE GUIDELINES
- Request Process
- To request a replacement diploma, students must complete and submit the Replacement Diploma Request Form, which is available on My ZSC.
- The completed form must be submitted to the Registrar’s Office along with the required payment.
- Name on Replacement Diploma
- The name printed on the replacement diploma will be the same as the name on the original diploma, unless the student has submitted a Change of Personal Information form.
- If the student has legally changed their name, they must submit the Change of Personal Information form along with legal documentation of the name change.
- Signatures and Diploma Marking
- The replacement diploma will bear the signatures of current college officials, not the officials from the time of the original diploma.
- The term “Official Replacement” will be printed at the bottom of the replacement diploma.
- Fees and Processing Time
- There is a fee associated with the issuance of a replacement diploma.
- Students should allow approximately four weeks for delivery of the replacement diploma once the request is processed.
